Medical master domestic and foreign took the lead in their own fields and academic exchanged without reservation


  On the morning of May 4, 2019, the "Main Event" of the Second Annual Meeting of Chinese Association of Microsurgeons (CAMS2019) - "The Master's Forum" was officially opened. The well-known experts and scholars at home and abroad in the field of microsurgery attended this academic lecture, which represented the highest level of microsurgery discipline.

  Academician Zhong Shizhen introduced the basic clinical anatomy of the anatomical specimens of microsurgery. By introducing traditional anatomical specimens and artificial pipe cast specimens, Academician Zhong emphasized that the distribution of blood vessels in various parts of the human body is regular and different. What’s more, the academician recommended that the clinician should master the anatomical shape and combine the surgical needs to improve the surgical technique.

  In the clinical application and localization research of sacral nerve anterior root electrical stimulator, Professor Hou Chunlin of Changzheng Hospital of Naval Military Medical University focused on the technology of bladder electrical stimulation urination. Due to the spinal cord injury above the cone, the bladder had a double barrier to urinary urination. He demonstrated through animal experiments that the bladder was completely detached by cutting off the S2S4 dorsal root, so the bladder became a flaccid bladder to solve the bladder storage function, ultimately, the bladder urinated by electrical stimulation.

  Dean of the Xijing Orthopaedic Hospital of the Air Force Military Medical University introduced the general principle of the repair strategy and treatment plan for the large segmental bone defect. The content were that the autologous bone or allogeneic bone transplantation was usually used when the bone defect is 5cm, the llizarov traction was preferred when the bone defect was accompanied by the limb deformity technique,  Masquelet induced membrane technique was preferred for infective long bone defects, 3D printed titanium alloy prosthesis was mainly used for the repair of pathological long bone defects, long bone defects with large soft tissue defects can be selected for bone-skin  (muscle) flap transplantation, and bone grafts with blood donor supply was mainly used for the repair of long bone defects with poor blood supply conditions. 

  Early microsurgery was still in its infancy, and there were many problems such as the lack of suture materials and fine surgical instruments, incomplete sewing technique and big wound caused by open surgery. The son of Harry Buncke who was the father of the world's microsurgery, Professor Gregory Buncke of the Buncke Clinic in the United States introduced his father's multiple inventions, which made microsurgery more delicate and minimally invasive and the surgical techniques constantly improving.

  In the treatment of brachial plexus injury reconstruction classification and facial paralysis repair, the lecture winner of 2019Harry J.Buncke, Chairman of the 9th Society for the Reconstruction of Microsurgery, Professor Zhuang Chuiqing from Linkouchanggeng Hospital of Taiwan China shared his experience. With his clinical experience, he encouraged clinicians to become familiar with surgical techniques,to be full of interest and curiosity, to hold the courage to create and challenge ideas, to be fully integrated the basic research and clinical research.

  On the progress of microsurgical treatment of avascular necrosis of femoral head, professor Zhao Dewei introduced the basic research, blood supply and clinical transformation of medical biomaterials in detail. In terms of the arterial ischemia stage of femoral head necrosis, he introduced in detail the principles and methods of head preservation therapy, including the original transect trochanteric bone flap therapy, the design of porous tantalum bar combined with bone flap therapy, and the reconstruction of femoral head with multiple bone flap combined with bone transfer technology.

  The treatment of femoral neck nonunion had always been an orthopedic problem. Former Chairman of the Microsurgery Branch of CMA Zhang Changqing, vice president of the Shanghai 6th People's Hospital, introduced the long-term follow-up study on the application of free iliac bone grafting. He highlighted three influencing factors, including age (the younger the better effect), the bone mass of the femoral neck before surgery (NSR>60%) and the internal fixation method according to the actual situation.

  Editor-in-Chief of Microsurgical Surgery (Microsurgery), Professor ZhangFeng, Distinguished    Professor of Zhongshan Hospital affiliated to Dalian University, reviewed the history of microsurgery. He hoped that the younger generations should learn from history and stand on the shoulders of giants to reform and innovate.

  Expert of Microscopic trauma orthopedics, Professor Liu Xiaolin, First Affiliated Hospital of Sun Yat-sen University,had detailed introduction Biomimetic Manufacturing and Engineering Anatomy.

  Professor Derek M.Culnan, an American specialist in reconstructive plastic surgery, talk about burns and reconstruction in the United States. He showed the current status of wound repair in the United States by introducing detailed cases, especially surgical images.  

  With the continuous update of microsurgery, microsurgery was entering a new era of super microsurgery Chairman of the 2019 world society for reconstruction microsurgery, President of the international super microsurgery society, professor Isao Koshima of the university of Tokyo Japan, described the cell surgery in which the nerve flap was not visible.

  In order to achieve a good prognosis in the reconstruction of head and neck defects, Dr. Eric Santamaria Linares, President of the world society of microscopic reconstruction surgery and professor of the national autonomous university of Mexico in 2021, emphasized the need to fully understand the challenges of treatment and to obtain new techniques of composite free tissue transplantation through continuous improvement and careful planning through multidisciplinary collaboration.

  As for the potential clinical application of new angiogenic factors in bone microenvironment, professor xu jiake, former President of Australian and New Zealand orthopedic research society and professor of medicine of the university of western Australia, Australia, said that they were involved in the fields of osteonecrosis, traumatic osteoarthritis, PTH treatment, tissue engineering and microsurgery.

  In the heel skin soft tissue and with microscopic repair of bone defect, the Chinese medical association branch of microsurgery, director of the committee member, president-elect,Affiliated orthopedic hospital 920th hospital of PLA joint logistic support force professor Xu Yongqing pointed out that to repair the soft tissue needs certain thickness, the width of the foot reconstruction needs certain, repair of soft tissue required sensory nerve, and the patient need heel care.

  At the meeting, the experts and scholars at home and abroad gave wonderful speeches on the theory and related clinical technology in the field of microsurgery without reservation, which benefited the participants a lot and left much to be wished for.
